Person Centred Practices
Alternative Futures Group takes a proactive and innovative approach to identify and plan support both for and with our service users. Detailed needs assessment helps us to meet the needs, aspirations and preferences of individual service users, whilst ensuring that we remain conscious of the impact that different backgrounds and cultures (including ethnicity and religion) may have.

Each of our services has a trained Person Centred Planning Facilitator, who can help to develop the plan ensuring that –
- Each individual service user is at the centre of the plan
- Family and friends are partners in the planning
- The plan reflects what is important to the individual, and the support they receive
- The plan helps that individual to build relationships and connect into their local communities
- Everyone involved in the work is committed to action
Our staff are trained in a number of person centred planning tools, including
- Essential Lifestyle Planning
- PATH
- MAP
.. helping people to think about what they want in their lives, to look at the support they need to pursue their aspirations, and to bring the people who can find the solutions for them together.
